Questionnaire structure

Five sections give specialists the context they need.

You can prepare the information before starting and upload available records in section four.

Prepare your records

Original imaging data supports a more useful first review.

DICOM original imaging files are preferred to report-only PDFs. Please include recent CT, MRI or PET-CT data where available, together with laboratory reports, pathology, genetic testing, prior surgery records and discharge summaries.

If your imaging study is too large for the website upload limit, submit the questionnaire first. Our team will arrange the next transfer step.

Initial medical questionnaire

Share the case for preliminary review.

Please complete the fields as fully as possible. If an answer is unknown, say so rather than guessing.

01Patient information

Tell us who the patient is and how our coordination team should make contact.

02Current medical condition

Describe the patient’s main condition and current daily status.

03Previous diagnosis and treatment

This is the most important section for specialist review. Include dates and outcomes wherever possible.

04Medical records and imaging

Original DICOM imaging data is strongly preferred to report-only PDFs. Text reports alone may limit specialist assessment.

PDF, DICOM, JPG or PNG. Up to 5 files, 10 MB each and 25 MB total. For larger imaging studies, submit the questionnaire first and our team will arrange the next transfer step. Temporary files are removed from the web server after processing. No files selected.

05Care goals and travel conditions

Help us understand the patient’s priorities and practical conditions for travelling to China.
